Windows (Explorer) Comment: is it metadata?

The only trick here is to determine what the tag is called in ExifTool.  Read FAQs 2 and 3 for help here.

Once you know the tag name, the basic command syntax is

exiftool -TAG="VALUE" FILE

or, as a specific example

exiftool -comment="this is a comment" c:\pictures\image.jpg

And I started a new forum thread to explain the basics of running from the command line in Windows.

...where DIR is the name of a directory/folder containing the images.  On Mac/Linux, use single quotes (') instead of double quotes (") around arguments containing a dollar sign ($).

Under Windows 8, adding a comment there gets added to the XPComment tag. 
* Did you read FAQ #3 and use the command listed there?
* Please use the Code button for exiftool code/output.
 
* Please include your OS, Exiftool version, and type of file you're processing (MP4, JPG, etc).

In XP the tags are added as:
XPTitle
XPComment
XPAuthor
XPKeywords
XPSubject

This, logically, should be the same for later versions (but then... it is Microsoft). :)
