I note there is a recent update that allows for writing to the MacOS MDItemUserTags.

I would like to create tags based on the Keywords within an image.

Does anyone know the correct syntax to achieve this?

I don't have a Mac but it should be like copying any other tag. You can check the MacOS tag names (https://exiftool.org/TagNames/MacOS.html) for all the names. Also, check to note under MDItemUserTags to make sure you have what it needs to work.
exiftool '-MDItemUserTags<Keywords' FileOrDir
