I am running Windows XP SP2. I tried to run ExifTool 7.76 (May 20 2009) from http://www.exiftool.org/exiftool-7.76.zip and got this:
Code:
C:\Documents and Settings\John Beale>exiftool
File/Spec/Win32.pm did not return a true value at C:/Perl/lib/File/Spec.pm line
21.
Compilation failed in require at C:/Perl/lib/IO/File.pm line 134.
BEGIN failed--compilation aborted at C:/Perl/lib/IO/File.pm line 134.
Compilation failed in require at -e line 313.

I do not have a C:/Perl directory, I thought this was supposed to be stand-alone.
Note: ExifTool 7.74 does work OK on this machine, so I rolled back to that one.

Please try deleting your temporary files (they should be found in a directory
like c:\Documents and Settings\USER\Local Settings\Temp\par-USER\exiftool-7.76,
where USER is your user name).  Then re-download and re-run exiftool 7.76.

I'm thinking that there must be something wrong with your installation, since
I didn't change anything that should affect this since version 7.74.

Thanks.
