What are history instances particularly when they are date stamped prior to the document's create date?  Does this mean that the current document is a derivative of an earlier document identified in the history instances? 

Is there a maximum number of history instances such that all of the history instances are not displayed in ExifTool?

I can't answer this, but you might have more luck asking this in a forum for the software that writes this metadata.

...where DIR is the name of a directory/folder containing the images.  On Mac/Linux, use single quotes (') instead of double quotes (") around arguments containing a dollar sign ($).
